ACT for Specialized Mental Health Treatment

The Assertive Community Treatment (ACT) Team, made of qualified mental health professionals, is our most intensive service provision team with the goal of providing specialized mental health treatment that supports an individual’s ability to remain in the community by identifying and addressing their personal recovery goals.

A person receiving ACT services will see their psychiatric provider for:

  • In-home medication management, monitoring and administration weekly and/or daily as determined by the psychiatric provider
  • Evidenced-based skills training and education with use of the Illness and Recovery Management Curriculum

To be eligible to receive ACT Team services, people must:

  • Be 18 years or older
  • Have a diagnosis of Schizophrenia, Bipolar Disorder or Major Depressive Disorder, each with psychotic features
  • Have recent psychiatric hospitalizations
