- Abuse
- Illegal Activity or Wrongdoing
- Employee Misconduct or Violations of StarCare Specialty Health System’s (StarCare) Code of Conduct by any member of StarCare’s workforce, report your concerns.
WHY REPORT
Health Care Fraud is a crime. Everyone has a responsibility to prevent fraud, waste and abuse in our health care system. StarCare is committed to assuring our own compliance. We maintain toll-free compliance hotlines to receive confidential reports from anyone who is aware of Fraud, Waste or Abuse, Illegal Activity or Wrongdoing, or of a violation in the company’s Code of Conduct.
REPORTING IS EASY AND CAN BE MADE ANONYMOUSLY
Describe Your Complaint/Concerns in Detail
Provide information on the type of violation or misconduct – who, what, when, where, and time.
If you have identified yourself in your complaint/report, a compliance staff member may contact you for additional information. However, if you are not contacted, this does not mean your complaint is not being investigated.
Your information remains confidential. Your identification and contact information will be kept confidential to the extent permitted by law.
